Inpatient Unit of the National Network of Continued Integrated Care that is articulated with the hospital of Agudos to provide clinical care, rehabilitation and psychosocial support, by clinical situation resulting from the recovery of an acute process or decompensation of chronic pathological process, to people with temporary loss of autonomy potentially recoverable.

The period of hospital stay is expected to be longer than 30 days and shorter than 90 consecutive days, for each admission.

The users of the Medium-Duration and Rehabilitation Unit are patients coming from other RNCCI responses, from the Hospital de Agudos and from home.

Referrals to the UMDR are made through the EGA (discharge management team) at the Hospital de Agudos, from home through referral by the Health Centre physician, and may also be referred by other RNCCI typologies.
